Preparation for your appointment
The passion and the vision of Deep Kinnections is to facilitate the healing process by taking a more clinical approach to massage and bodywork through the use of proper evaluation prior to and after treatment.
On your first appointment it is important that we take our time to gather all the necessary information needed to ensure that we are meeting your insurances requirements for medically necessity. Please have available the following items:
Insurance Card and photo ID
Any prescription or referral that you may have
Intake Paperwork (download here). You can email your forms ahead of time if you wish
Be prepared to pay any portion of your patient responsibility at the time of service (Deductibles, Co-pays, Coinsurance, etc*)
Please note that your initial evaluation includes reviewing your paperwork and functional rating scores, documenting and creating your short and long term goals, muscle testing and/or palpation exam, and posture analysis.
This is a crucial step that cannot be rushed - this will take about 15 minutes.
As a part of office policy, re-evaluations will occur according to your prescription or at a minimum of every 6-8 weeks - make sure to check your insurance to see if evaluations and re-evaluations are a covered benefit on your plan**.
*Medical Massage Services are billed at $35 per unit

Insurance continues to become more and more complicated so things can get confusing but knowing and understanding how your benefits work can be you biggest advantage. HERE is a list of questions to ask when calling your insurance to make sure you are covered for massage therapy - please bring this with you on your first appointment.
